Community Elementary School, 102 S. Cline Road
Time: Wednesday, August tba - 8 a.m. to 6 p.m.
Thursday, August tba - 8 a.m. to 6 p.m.
Enrollment set up for all new and returning students will be
in the multi-purpose room/cafeteria, north end of school.
All students P-K through 12 will enroll these two days
School nurse will give free immunizations to those children in need.
Free and reduced lunch applications will be available.

a student is enrolling for the first time in U.S.D. 445:
- State Birth

Records - It is mandatory that students entering
kindergarten have the following: 2 MMR's; 4 DPT's; 4 Oral
Polio Vaccines - One of the DPT's and one of the oral
polio immunizations must have been received after the
child was four years old. All children who will be 11
years old by August 31, 2003, must have a second MMR
prior to entering school. Students entering grades 7-12
must have a 10-year DPT booster, if needed.
- Complete Health
Assessment for Students 5-8 years - This includes a
health history, physical examination and hearing and
vision screening. Health Assessments and forms may be
obtained through your local physician or the Montgomery
County Health Department, 908 South Walnut, Coffeyville.
Students should have this health assessment prior to the
first day of school.

There is an additional $10 instructional materials fee required for
grades PK-8 and $12 for grades 9-12.

Bus Transportation - Bus routes will begin August
21. Participation in the school bus program is limited to those
students who live 2 1/2 miles or more from their attendance centers and
shuttle buses for reassigned students. Contact 251-8441 for
questions concerning bus transportation. Shuttle buses -
Shuttle buses will run daily from three pick up points to the Community
Elementary and return to the pick up points after school. Students
may board the buses from the pick up points at the following times:
Whittier
7:20 a.m.
Lowell
7:15 a.m.
Longfellow 7:15 a.m.
The Shuttle is not for children in grades 7-12.
